ForgeKit OS History Deck — External Review Fixes and the AI-in-the-Loop Image

Applied a real external review's verified corrections to the history deck's content, and generated a deliberately inverted 'AI in the loop' visual (a wielded tool, not a caged machine) to seed a future epilogue.
