supersedes field on spin_triage.accepted, corrected to use the same typed {type, ref} reference shape

patterns.md supersedes convention aligned to the same typed-reference shape (documented in prose, since patterns.md has no JSON structure)

session_audit.outcome_match — the one genuinely new field, replacing an earlier bare outcome_matched_expectation boolean with an assessment + evidence structure, gated on shaping_pass.outcome having actually been populated pre-build

The ideas are good. The current state is a bridge with all the beams placed but no bolts inspected. I would not drive the OS across it yet.
